To establish a credential file, please follow the links below. The general forms are in a "print only" format and cannot be saved or submitted on-line. Print the forms and deliver or mail to Career Services, 2112 Cleveland Blvd, Caldwell ID 83605. Forms may also be submitted by fax: 208-459-5849.
