SEXUAL MISCONDUCT INCIDENT REPORT

Submit Report

For reporting sexual misconduct, sexual assault, stalking, dating violence, and/or bias, discrimination, harassment, violence or bullying on the basis of sex, gender, sexual orientation, gender identity, gender expression and pregnancy/parenting. Such reports are sent directly to and reviewed and investigated by the Title IX Office.

BIAS INCIDENT REPORT

For reporting non-sex-based and non-gender-based bias, discrimination, harassment and/or hate crimes. Such reports are sent directly to and reviewed and investigated by the Bias Assessment & Response Team (BART).

CAMPUS SAFETY INCIDENT REPORT

For reporting concerns of suicidal ideation; a safety concern, such as fire or motor vehicle accident; violations of the College’s alcohol, drug and weapons policies; or criminal activity, such as threatening or obscene behavior, fraud, theft, hazing or bullying. Such reports are sent directly to and reviewed and investigated by the Office of Campus Safety, in consultation with the Vice President of Student Affairs.

It is important to note that you may choose to submit the report anonymous or non-anonymously. If you choose to report anonymously, do not include your name or contact information.

Confidentiality will be maintained to the extent allowed under policy and law. Information is disclosed to the individuals or groups mentioned in the report when allowed by college policy.
